Physicians who wish to make a referral directly to either Dr. Wilbur at (707) 546-7661, drwilbur@northbaymedpsych.com or Dr. Liguori at (707) 527-0260, drliguori@att.net. Please include a brief description of the reason for the referral and how you think we may best assist your patient in their recovery.
For certain types of insurance (e.g., Worker’s Compensation) a written referral on professional letterhead or a prescription form is required. A request for pre-authorization for psychological services originating from the Primary Treating Physician’s office may be required as well. A written statement regarding such referrals may be sent via email, followed by a signed referral document sent by mail. Our mailing address is P.O. Box 2249, Guerneville, CA 95446.
We encourage physicians to provide patients with our contact information when making a referral. We also request that the patient contact us directly to make an appointment.
